- Define the following terms: Polypharmacy, Duplicate Therapy and Pill Burden
- List rationale for minimization of medication use in end-of-life care
- Formulate medication use criteria for select medications
- Identify therapeutic alternatives for select medications
As individuals approach the end-of-life they often desire to minimize the use of medications and prescribers are often asked by patients or caregivers to discontinue medications used to manage chronic disease states. Clinician’s often struggle with constructing a rationale medical plan to deal with medication use at the end-of-life. This presentation will address the issues surrounding minimization of medication use in end-of-life care and propose a rationale approach to managing the dilemma of “to continue or to discontinue” select medications in end-of-life care.
